Care after fractional laser. What to recommend to the patient — a protocol for the clinic

A fractional laser — ablative or non-ablative — works through controlled damage: it creates thermal microcolumns in the skin that trigger collagen remodelling. The price of this mechanism is a temporary inflammatory state, erythema, swelling and massive production of free radicals. How the skin gets through the first 72 hours largely determines the final result of the therapy.

Day 0: immediately after the treatment

Skin after a laser is overheated and the epidermal barrier is open. The priority is calming, protection against water loss and neutralising oxidative stress at the source.

  • touch-free application — a mist instead of a cream applied with fingers
  • antioxidant ingredients neutralising the wave of free radicals (ROS)
  • no acids, retinoids, alcohol or fragrances
  • only indirect cooling — never ice directly on the skin

Days 1–7: regeneration and barrier rebuilding

In this phase the skin rebuilds the hydrolipid barrier and closes the micro-damage. The patient continues a simple, repeatable routine at home: gentle cleansing, an antioxidant mist several times a day, a barrier-rebuilding preparation and absolute SPF 50+ photoprotection.

What to avoid absolutely for 7–14 days

  • sun exposure and tanning beds — risk of post-inflammatory hyperpigmentation
  • sauna, swimming pool, intense exercise — overheating and microbiology
  • AHA/BHA acids, retinoids, mechanical scrubs
  • makeup on skin that has not fully closed
  • touching and picking at the flaking epidermis
